Your initial appointment can be either a consultation which includes evaluation and discussion on treatment options or both a consultation and treatment. A parent or guardian must accompany a patient under 18 at the visit.

Please bring the following at your first visit:​

• Photo ID

• A completed referral form if applicable

• Your dental insurance if applicable

• Your pharmacy information and a list of medications you are currently taking if applicable
